Entry and Submission Guidelines
Each Entry MUST:
- Be submitted in a standard three-ring binder no larger than two inches
- Include a completed entry form with payment.
- Include a cover with: Company Name, Contact Information, Project Name, Category Number
- Include a summary, no more than two 8½” x 11” typewritten pages, 10-12 point font, double-spaced. Include:
- Planning and goals of project
- Implementation of the project
- Results and evaluation of the project
- Budget (demonstrate how effectively you used the funds)
- Include any support material needed (Clearly labeled with Contents Description, Project Name and Category Number)
- Include a CD or Flash Drive with:
- 30-word summary of the project

Image of the project (for use in the awards ceremony)
Please follow these directions closely and completely. Judges reserve the right to disqualify entries that are incomplete or submitted incorrectly. CHC is not responsible for lost or misplaced supporting material.

Submission Guidelines
The Gold Leaf Awards are open to CHC members and non-members. Projects of all budgets and sizes are welcome and will be judged based on the summary and supporting materials that thoroughly show:
- Planning and goals of project
- Implementation of the project
- Results and evaluation of the project
- Budget (demonstrate how effectively you used the funds)
In addition, the panel of judges representing hospital, non-profit organizations and marketing and public relations professionals will score each entry based on the category-specific requirements. Judges reserve the right to re-categorize entries. All scoring decisions are final.
Awards will only be given to those entries meeting the scoring criteria, and no more than three awards will be given per category.
Eligibility and Entry Requirements
- Any Colorado healthcare organization
- Any in-state or out-of-state agency creating work for a Colorado healthcare organization
- Any in-state agency creating work for an out-of-state healthcare organization
- There is no limit to the number of entries an individual or organization can submit
- Entries must consist of original work created between August 2011 and August 2012
- All entries, including special award nominations, are due by TBD
