Entry and Submission Guidelines

Submission Guidelines
The Gold Leaf Awards are open to CHC members and non-members. Projects of all budgets and sizes are welcome and will be judged based on the summary and supporting materials that thoroughly show:
· Planning and goals of project
· Implementation of the project
· Results and evaluation of the project
· Budget (demonstrate how effectively you used the funds)
In addition, the panel of judges representing hospital, non-profit organizations and marketing and public relations professionals will score each entry based on the category-specific requirements. Judges reserve the right to re-categorize entries. All scoring decisions are final.
Awards will only be given to those entries meeting the scoring criteria, and no more than three awards will be given per category.
Eligibility and Entry Requirements
· Any Colorado healthcare organization
· Any in-state or out-of-state agency creating work for a Colorado healthcare organization
· Any in-state agency creating work for an out-of-state healthcare organization
· There is no limit to the number of entries an individual or organization can submit
· Entries must consist of original work created between August 2009 and August 2010
· All entries, including special award nominations, are due by August 27, 2010.
Please follow these directions closely and completely. Judges reserve the right to disqualify entries that are incomplete or submitted
incorrectly.
Complete entry form and attach it to your submission binder. You may make copies of the entry form to accommodate

Each entry must be submitted in a standard three-ring binder no larger than two inches. The binder cover must include:
~ Company name
~ Contact information
~ Project name
~ Category
Binders must contain and securely fasten all supporting material including photos, letters, clippings, presentations, DVDs,
CDs and A/V tapes. Multimedia must be clearly labeled with the company, contact information, project name and category.
CHC is not responsible for lost or misplaced supporting material.
?Projects MUST include a summary. Summaries should be no more than two 8½” x 11” typewritten pages, 10-12 point font,
double-spaced. Include the following:
~ Planning and goals of project
~ Implementation of the project
~ Results and evaluation of the project
~ Budget (demonstrate how effectively you used the funds)
Projects MUST include a CD containing a brief, 30-word summary AND a jpeg image of the entry to be used during the awards
ceremony. Save the text and image on the CD, please.
Duplicate awards may be purchased after the awards ceremony.
