Entry and Submission Guidelines

Any Colorado Healthcare Organization
Any In-State or Out-of-State Agency Creating Work for a Colorado Healthcare Organization
Any In-State Agency Creating Work for an Out-of-State Healthcare Organization

Entry Requirements

There is no limit to the number of entries an individual or organization can submit.
Entries must consist of original work created between August 2007 and August 2008.

Submission Guidelines

A completed entry form must be attached to each entry. You may make copies of the entry form to accommodate multiple submissions.
Projects need to include a summary. Summaries should be no more than two 8.5” by 11” typewritten pages, 10-12 point font, double-spaced. Summaries should include the following:
- Planning/Goals of Project
- Implementation of the Project
- Results/Evaluation of the Project

Each entry must be submitted in a standard 3-ring binder no larger than 2”. The binder cover must included company name, contact information, project name, and category.
Binders must contain all supporting material including photos, letters, clippings, presentations, DVDs, CDs, and A/V tapes. Multimedia must be clearly labeled with the company, contact name, project information, and category.
Finalists will be notified and asked to email a brief summary and jpg image of the entry to be used during the awards ceremony.
All supporting material must be secured in the binder. CHC will not be responsible for lost or misplaced support material.
